Printable PCOS tracker and planner

What it is

A printable cycle and symptom tracker for one month: a cycle log for the start and end date and flow of each period, a daily check sheet with rows you name yourself and a box for every day, a month overview, and a notes-for-my-appointment sheet with room for your questions. It is a blank record: there is no pre-filled symptom list, and nothing is scored, interpreted or judged.

What's in the pack

Using it

  1. In the builder, pick PCOS tracker and choose the month.
  2. Type a name for each row you want to keep track of (up to 12), or leave the rows blank to write them in by hand.
  3. Print the pack. Write each period's dates on the cycle log and mark the daily sheet once a day.
  4. At the end of the month, count the days on the overview. Fill in the appointment sheet before a visit and take it with you.

Print it

Every sheet previews free in the builder. Printing is $7 once (prints for 31 days, nothing renews) or $5 a month if you want fresh sheets every month. No account: your printing key is emailed to you. The same key prints every Streaksheet pack.

Secure checkout by Stripe. Print on paper or save as a PDF, from a computer or a phone. Already paid? Enter your key in the builder.

Questions

Is the PCOS tracker free?

Building and previewing it is free; the preview has a watermark. Printing costs $7 once (31 days of printing, no renewal) or $5 a month.

Does it come with a list of symptoms?

No. Every row on the daily check sheet is blank. Name the rows yourself in the builder, up to 12, or leave them blank and write them in by hand after printing.

What if my cycles are irregular?

The cycle log has no set cycle length. It is one line per cycle, 13 lines to a sheet, for the start date, end date and flow you write yourself.

Does it score or interpret what I write?

No. It is a blank record. Nothing is added up for you except the day counts you write on the month overview yourself.

Can I use it as a plain period tracker?

Yes. The sheets are blank, so the cycle log and month calendar work as a printable period tracker whatever you are keeping track of.

Can I get it as a PDF?

Yes. In the print dialog, choose Save as PDF. It is not a fillable PDF: you fill it in with a pen after printing.

What paper size does it print on?

The sheets are laid out on an 8.5 × 11 inch page; the daily check sheet prints in landscape. The paper size itself comes from your print settings, so check the print preview if you use A4.

A record-keeping sheet, not medical advice.
