Printable menopause symptom tracker and diary

What it is

A daily symptom diary for one month: the things you want to keep track of down the left (hot flashes, night sweats, sleep, mood and your cycle to start, renamed or added to as you like) and a box for every day of the month. It comes with a month overview and a notes-for-my-appointment sheet. It is a blank diary: nothing is scored, interpreted or judged.

What's in the pack

Using it

  1. In the builder, pick Menopause diary and choose the month.
  2. Keep the five starting rows, rename them, or add your own (up to 12).
  3. Print the pack and mark the grid once a day. A tick works, or a number from your own key.
  4. At the end of the month, count the days on the overview. Fill in the appointment sheet before a visit and take it with you.

Print it

Every sheet previews free in the builder. Printing is $7 once (prints for 31 days, nothing renews) or $5 a month if you want fresh sheets every month. No account: your printing key is emailed to you. The same key prints every Streaksheet pack.

Secure checkout by Stripe. Print on paper or save as a PDF, from a computer or a phone. Already paid? Enter your key in the builder.

Questions

Is the menopause symptom tracker free?

Building and previewing it is free; the preview has a watermark. Printing costs $7 once (31 days of printing, no renewal) or $5 a month.

Can I change what it tracks?

Yes. The five starting rows are hot flashes, night sweats, sleep, mood and period / cycle. Rename or remove any of them and add your own, up to 12 rows.

Does it score or interpret what I write?

No. It is a blank diary. Nothing is added up for you except the day counts you write on the overview yourself.

Does it work for perimenopause?

Yes. It is a blank diary, so it works for any stage. Name the rows for whatever you want to keep track of.

Can I get it as a PDF?

Yes. In the print dialog, choose Save as PDF. It is not a fillable PDF: you fill it in with a pen after printing.

What paper size does it print on?

The sheets are laid out on an 8.5 × 11 inch page; the daily diary prints in landscape. The paper size itself comes from your print settings, so check the print preview if you use A4.

A record-keeping sheet, not medical advice.
