What's in the pack
- Blood pressure log: one portrait page per month, a dated row for every day, with one or two readings a day (morning and evening), each with time, systolic, diastolic and pulse, plus a notes column.
- Medication checklist: a landscape grid with your medications down the left (name, dose and time as you type them) and a box for every day of the month.
- Refill tracker: one row per medication for the Rx number, the date it was last filled, refills left, the next refill date and an ordered box, with space for your pharmacy name and phone.
Using it
- In the builder, pick Health log, choose the month and whether you write down one or two readings a day.
- Type your medications with the dose and time, copied from the labels. They print on the checklist and the refill tracker.
- Print the pack and keep the log next to your monitor. Write each reading down as the monitor shows it.
- Print a new month when the sheet is full. The dates and weekdays change with the month you pick.

Every sheet previews free in the builder. Printing is $7 once (prints for 31 days, nothing renews) or $5 a month if you want fresh sheets every month. No account: your printing key is emailed to you. The same key prints every Streaksheet pack.

Questions
Is the blood pressure log free?
Building and previewing it is free; the preview has a watermark. Printing costs $7 once (31 days of printing, no renewal) or $5 a month.
Does it tell me whether a reading is high or low?
No. It is a blank log for writing readings down. It has no ranges, no colour bands and no calculations.
Can I log two readings a day?
Yes. Choose two readings a day in the builder and each date gets a morning and an evening set of columns. Choose one if you only write one down.
How many medications fit?
Up to 10 on the checklist and the refill tracker. Blank rows print empty so you can add one by hand.
Can I get it as a PDF?
Yes. In the print dialog, choose Save as PDF. It is not a fillable PDF: you fill it in with a pen after printing.
What paper size does it print on?
The sheets are laid out on an 8.5 × 11 inch page; the medication checklist prints in landscape. The paper size itself comes from your print settings, so check the print preview if you use A4.
